Tom Cullen
Tom Cullen is the co-founder and CEO of Community Care Options. He has a B.S. degree in Business Administration and has been a licensed administrator since 1980. During his career he has managed facilities from 6 beds to 200 apartments, has bought and sold five RCFE’s and managed facilities for Marriotts, Walker Methodist and other smaller private companies.
Adam Cullen
With more than 25 years working with Community Care Options and care facilities across California, Adam has dedicated his career to improving the quality of education and service in the care industry. He graduated from San Francisco State with a degree in Communication, focusing on Organizational Communication and Public Speaking. His lifelong passions for photography and technology came together when he began making 3D scans of care facilities to use in their marketing, and virtual reality based education. When he’s not working, you can find him spending time with his wife and son at the park or playing video games.
Greg Asher
Jennifer Schwab brings more than 30 years of experience working exclusively in the Residential Care and Social Services industry. She is deeply knowledgeable about the unique risks and insurance needs facing residential care and home health organizations.
Jennifer specializes in developing stable, comprehensive insurance programs that help organizations stay properly covered and confident in their protection. Her personalized approach and strong communication skills ensure clients receive competitive, effective coverage tailored to their specific needs.
Through her expertise in risk management and regulatory navigation, Jennifer helps organizations maintain compliance, reduce exposure, and create long-term stability.
Teena Hagerman
Tony Greer brings more than 20 years of experience leading dining services across assisted living, memory care, and skilled nursing communities. As Divisional Director of Dining Services for Sapphire Health Services, he oversees dining operations for ten skilled nursing centers, guiding budget management, menu development, procurement, and team training.
Previously, Tony led dining operations for 41 assisted living communities with Prestige Care, including 19 memory care locations, where he strengthened budgets, enhanced menu programs, and developed high-performing teams. His background also includes operational leadership roles in senior living, achieving full census, strong financial performance, and deficiency-free surveys.
A graduate of the California Culinary Academy, Tony is a Certified Dietary Manager and Licensed RCFE Administrator, known for his expertise in dining program development, dementia care, budget planning, and team leadership.
Spencer Rogers
Teena Hagerman brings more than 45 years of experience in the assisted care industry, dedicating her career to elevating care standards and supporting both residents and staff. Over the past 25 years, she has owned and operated her own assisted care communities while also serving as an Executive Director, Activity Director, compliance specialist, and staff trainer within large facilities.
For the past decade, Teena has focused on assisted care consulting, specializing in compliance and risk reduction. Her hands-on operational experience allows her to help organizations strengthen systems, improve outcomes, and limit liability.
As a member of the Community Care Options team, Teena mentors and educates administrators, delivering practical, engaging training that helps organizations stay confident and compliant.
Tony Greer
Spencer Rogers has been a seasoned professional in the senior care industry, specializing in workforce development, HR, and talent management since 2017. With ample background in education dating back to 2006 in both academic and professional settings, he brings a wealth of knowledge to Community Care Options. Here, he focuses on continuing education and consulting, helping to shape training programs to support the senior care workforce with his extensive experience.

Nancy Schier Anzelmo
Nancy Schier Anzelmo is a seasoned gerontologist with extensive expertise in senior housing and dementia care. As the owner of Alzheimer’s Care Associates LLC, Nancy consults on senior housing projects nationwide, collaborating with developers, providers, and organizations like the Alzheimer’s Association.
She’s a lecturer in the Gerontology Department at California State University, Sacramento, and the Co-Founder of the Connected Horse program, which offers equine-guided engagement for individuals affected by dementia.
Mark Cimino
Mr. Cimino served as a licensed Skilled Nursing Home Administrator as well as a licensed Residential Care Facility Administrator before eventually returning to work with CiminoCare. In 1994, he earned a Juris Doctor from the University of California, Hastings College of the Law. He is now the Chief Executive Officer and an owner of CiminoCare, responsible for the entirety of the business operations, including oversight of all communities, administrators, and finances.
Sandy Broeckel
Sandy graduated from Chico State in 1987. She spent 10 years in Med-Surg at Chico Community Hospital, 10 years in dialysis, occupational health, and Assisted Living and 10 years at The Feather River Hospice House as the nurse manager. After the Camp Fire and the closing of the Hospice House, she was blessed as Coordinator of Hospice and Palliative Care for Enloe Medical Center for 7 years and has now moved into Case Management. Her greatest gratification in her job is helping patients, family members and staff work through tough transitions and making it good.

Rachel O’Rourke
Rachel O’Rourke is a strategic marketing leader and business growth expert who has spent more than 15 years helping organizations scale through clear messaging, strategic systems, and authentic relationship-building. As a fractional Chief Marketing Officer, Rachel has supported the launch and growth of dozens of companies—guiding teams through branding, customer acquisition, digital transformation, and sustainable long-term marketing strategies.
Known for her practical, approachable teaching style, Rachel specializes in breaking down complex business and marketing concepts into simple, actionable steps. Her work centers on helping leaders understand their audiences, strengthen communication, and build meaningful customer experiences that drive retention, reputation, and revenue.

Erica Bosque

Katie Daniels
Katherine Daniels brings over 18 years of expertise in clinical education and rehabilitation, with a proven passion for mentoring and empowering both students and staff. She holds a Doctorate in Physical Therapy from the University of St. Augustine for Health Sciences and a Bachelor's in English Literature from the University of Florida. Katherine is APTA Board Certified in Geriatrics, a Certified Clinical Instructor, and has numerous certifications in fall prevention, dementia care, geriatrics, and is also a 200-hour registered yoga instructor.
Her career spans various leadership roles, including Director of Rehabilitation at Rockport Healthcare and Bayshore Rehabilitation, as well as Regional Manager with RehabCare. Katherine’s creativity, love for teaching, and dedication to improving care for the elderly make her a vital figure in the healthcare field. We are thrilled to have her share her insights with us today.
